- Indonesia’s national health insurance (JKN) has been launched since 2014 and reached 86% participation in 2021. Although currently Indonesia’s at demographic bonus period, the accelerated aging society growth will be inevitable healthcare issue in near future and should be addressed by the government. In the other side, central government has always been at the center of JKN discussion. But since decentralization in 1999, the bigger role of provinces is an interesting subject to be explored. Both elements will be discussed with comparative analysis to USA heathcare because their healthcare system has more than a century of experiences, failures, and reforms. JKN will be compared to Medicare, healthcare program for population aged 65 years and above while Indonesia’s provinces will be compared to the state government. The paper is concluded that BPJS Kesehatan should start to analyze the demographic profile and to distribute the risk to different methods such as premiums and government revenue. In relate with the role of provinces in Indonesia, the opportunities lie at provinces’ involvement to the JKN scheme which later can increase provinces’ power to design tailored health program. Hence, the capacity building is heavily needed in local bureaucrats.
